How to request a Non-Disclosure Agreement from us

A non-disclosure agreement is where I and those who work for me promise to treat your confidential invention information as a secret. We have a standard agreement, which you can request below, or a .pdf version that you can print-out yourself, sign, and fax or mail to us for signature.

Alternately, you may email or fax us your own NDA for us to review. To see our address, fax number, or email address, see our Contact Us page.

Complete the following:

* First Name:  * Last Name:
Company (if any):
* Mailing Address:

* City:
* State: Zip:
* Phone:

Direct Line or Ext:

Fax:
* Email:
*Invention Title:

You do not have to disclose the "secret" or "key innovative concept" in your title. Use a generic title that will allow identification of your particular invention once it becomes patented.

For example, instead of using a title such as "Combination Golf Glove and Divot Repair Tool" you might say just "Golf Glove."

Field of the invention:

For example, "Golf" or "Tools" or "Vehicles," etc.

* Delivery Method:

Fax First Class Mail Email

Select how you would like us to deliver the signed NDA to you.

How did you find our website?:

* Do you agree to the terms of the non-disclosure
agreement at left (type "yes"/"no")?
